The short answer: the best tours in Milan for a first visit are Leonardo’s Last Supper, which you cannot see without a booking, and the Duomo with its rooftop terraces. With a free day, add Lake Como, 64 minutes away by train, or a wine day in the Langhe. The tours we compare start from about $64 for the Duomo to about $208 for Barolo (checked September 2026).

Milan rewards planning more than any other Italian city, because its most famous sight has the strictest rules. The Last Supper is seen in 15-minute slots of 40 people, and every visitor needs a reservation. This plan tells you which Milan tours to book first, what the official tickets cost if you go on your own, and how to fit in the lakes. Each tour links to our own page, where we compare two options side by side.

Day 1: Leonardo’s Last Supper

Book this before anything else. The official ticket costs €15 for visitors aged 25 and over, booking is mandatory for everyone, and visits last 15 minutes with 40 people at most (checked September 2026). Official tickets go on sale in batches and sell out fast, so check the official site as soon as you know your dates. If they are gone, a guided tour includes a guaranteed ticket: our Last Supper tour page compares a 90-minute visit with a three-hour tour that adds the Duomo. For the story of the painting, read our guide to The Last Supper.

Day 1 afternoon: the Duomo and its rooftop

The Duomo is two visits in one: the cathedral and the terraces among its spires. On the official site, the cathedral and museum cost €10, the rooftops €18 by lift or €16 on foot, and a combined ticket €26 with the lift or €22 with the stairs; there is no online service fee (checked September 2026). Alternatively, a guided tour adds skip-the-line entry and context: compare the options on our Duomo and rooftop tour page.

Afterwards, walk through the Galleria Vittorio Emanuele II to La Scala, and, if you like art, go on to the Pinacoteca di Brera: €15, free with a reservation on the first Sunday of the month, open Tuesday to Sunday from 8:30 am (checked September 2026). More ideas in things to do in Milan.

Day 2: Lake Como or the Langhe

The lakes are the reason many travelers stay a second day. Regional Trenord trains run from Milano Centrale to Varenna in about 64 minutes, once an hour, from €7.40 in second class (checked September 2026). However, the boats, villas and connections take planning, which is why the Lake Como day trips from Milan are among the best tours in Milan for a first visit. For wine instead, the Barolo wine tours include a day trip from Milan to the Langhe hills.
