This is one of the most booked experiences in Florence and the reason is simple: getting to Siena, San Gimignano and Pisa on your own in a single day is possible only if you are willing to spend it reading timetables. Here someone else drives, and a guide stays with you the whole way.
What you are trading is the day itself. Check-in is at 7:45 in the morning and you are back in the evening, on a coach, in a large group. If that sounds like the opposite of a Tuscan holiday, it is — and it is still the most efficient way to see three hill towns that are nowhere near each other.

What twelve hours actually means
Check-in is at 7:45am in Piazza della Stazione, by the taxi rank, where the guide holds a Walkabout Tours sign. From there it is Siena with a local guide, San Gimignano, lunch and wine at a winery, and Pisa, with driving between each. The stops are real but they are not long: this is a highlights day, and the towns are being introduced to you rather than explored.
The provider states it directly: book any rail travel out of Florence after 9pm, to leave room for traffic on the way back. If you were planning to do Tuscany in the day and move on to Rome or Venice that evening, this trip does not fit — and a missed train costs more than the tour.

How it fits into a Florence trip
Put it on a full day in the middle of your stay, never on an arrival or departure day. It pairs badly with anything else: after twelve hours you will want dinner and bed, not a second booking. It pairs well with a slower Florence day either side, when the city itself is enough.

Can I catch a train the same evening?
The provider recommends booking rail travel out of Florence only after 9pm, to allow for traffic on the return. Treat anything earlier as a risk.

Do we go inside the Leaning Tower?
No. Pisa is a stop on the day, and climbing the tower is a separate ticket.
